Choosing between AbbVie Inc (NYSE:ABBV) and Johnson & Johnson (NYSE:JNJ) requires balancing the potential for aggressive pharmaceutical growth against the stability of a diversified medical conglomerate.

AbbVie is a focused biopharmaceutical player known for its dominance in immunology, whereas Johnson & Johnson operates a broader business model following its separation from its consumer health division. Investors often compare these two when looking for reliable dividends and exposure to the healthcare sector.

The case for AbbVie

Within the landscape of pharmaceutical stocks, AbbVie focuses on discovering and commercializing treatments for serious conditions in immunology, oncology, and aesthetics. The company relies on three major wholesale distributors, McKesson Corp (NYSE:MCK), Cardinal Health Inc (NYSE:CAH), and Cencora Inc (NYSE:COR), which account for almost all of its U.S. sales. Customer concentration like this adds a layer of risk to the business because a disruption with any single partner could impact distribution.

In FY 2025, revenue reached nearly $61.2 billion, representing a growth of roughly 8.7% over the previous year. The company generated a net income of approximately $4.3 billion during this period. This resulted in a net margin of close to 7%, which measures the percentage of revenue remaining as profit after all expenses are paid.

As of its December 2025 balance sheet, the company reported a debt-to-equity ratio of -21x, indicating that its total liabilities exceed its shareholders’ equity. The so-called current ratio, which measures the ability to pay short-term debts with short-term assets, is approximately 0.7x. Despite these figures, the business generated more than $17.8 billion in free cash flow, which is the cash left over after the company pays for its operating costs and capital investments.

The case for Johnson & Johnson

Johnson & Johnson operates through two primary segments: Innovative Medicine and MedTech. Its largest product group, the Darzalex portfolio, accounted for approximately 15% of total revenues in 2025, providing a strong foundation for its pharmaceutical business. The company maintains a global reach, distributing its products directly to hospitals, wholesalers, and healthcare professionals in nearly every country.

In FY 2025, revenue reached approximately $94.2 billion, which was an increase of nearly 6% compared to the prior year. The company reported a net income of close to $26.8 billion during this fiscal period, a significant increase from $14.1 billion the prior year. This led to a net margin of roughly 29%, indicating the high level of profit the business generates from every dollar of sales.

As of its December 2025 balance sheet, the debt-to-equity ratio was roughly 0.6x, suggesting a conservative amount of debt relative to equity. Its current ratio is approximately 1.0x, showing that its short-term assets are roughly equal to its short-term liabilities. The company also produced nearly $19.7 billion in free cash flow, providing significant capital for research or acquisitions.

Risk profile comparison

AbbVie faces significant revenue concentration, as its products Skyrizi and Rinvoq account for approximately 42% of its total net revenue. The company is also navigating intense regulatory and pricing pressures, particularly from the Inflation Reduction Act, which affects reimbursements for key drugs. Furthermore, it faces risks from potential patent litigation and the inherent difficulty of successfully bringing new research projects to market.

Johnson & Johnson faces substantial competition across its medical segments and is sensitive to the loss of patent exclusivity for major products such as Stelara. The company remains involved in complex legal proceedings, including product liability claims related to its former talc products. Additionally, it must manage global supply chain disruptions and the complexities of integrating its business after the planned separation of its orthopedics division.

Valuation comparison

AbbVie currently trades at a lower Forward P/E than its peer, though Johnson & Johnson carries a lower P/S ratio relative to its total annual sales.

Metric AbbVie Johnson & Johnson
Forward P/E 18.4x 22.8x
P/S ratio 7.1x 6.6x

Which stock would I buy in 2026?

Johnson & Johnson is an overlooked behemoth compared to faster growers in the biotech sector. The business has 28 products and platforms that generate more than $1 billion in annual revenue. Its pharmaceuticals arm, Innovative Medicine, has had two FDA approvals this year and has two more approved drug product launches this year in the E.U. and U.S. The Innovative Medicine division is 65% of JNJ's annual revenue and appears to have a robust pipeline beyond those pending products. MedTech, its other arm, sells a wide range of healthcare products, including wound care, surgical instruments and implants, hip and knee replacements, and spinal implants. The recent buy of Atraverse Medical bolsters that business. Revenue for JNJ is expected to rise to $101 billion in the current fiscal year.

AbbVie is buying itself growth with its recent buy of Apogee Therapeutics. Its two relatively new immunology products, Skyrizi and Rinvoq, have proven to be true growth drivers for the business, and investors are hopeful that an FDA decision on a Parkinson’s treatment later this year will bode well for the company. AbbVie is expected to see screaming growth in net income this year, from $4.2 billion to $14.6 billion. Sales should grow to $67.2 billion.

Both these healthcare giants are fairly close in P/S ratio and forward price-to-earnings. AbbVie appears to have the higher upside, with a pipeline that some on Wall Street believe could include blockbusters (more than $1 billion in lifetime revenue) for treating schizophrenia, Parkinson’s disease, psychosis from Alzheimer’s, and certain solid tumors.

Not all of those may work out as hoped, but it’s impressive pipeline makes AbbVie the pick over Johnson & Johnson.
