Total surgical procedures grew 15% year over year in the third quarter.
Instrument price reductions will lower procedure revenue by 3% to 5%.
Intuitive placed 18 robotic systems in outpatient surgery centers during the quarter.
On its third quarter of 2026 earnings call, held on Oct. 20, 2026, Intuitive Surgical (NASDAQ:ISRG) announced planned price reductions on core instruments to drive broader procedure adoption across routine surgeries. Lowering procedure costs trades short-term revenue per case for long-term customer lock-in and expanded market volume. Three things management said stand out.
Management is extending instrument useful life and adding volume discounts to lower customer costs in routine procedures, which impacts instrument and accessory (I&A) revenue. This move sacrifices short-term revenue per procedure to expand total adoption in cost-sensitive outpatient settings.
This is a strategic decision to reinvest a portion of the value our innovation creates and lower customer costs where they will have the greatest impact on adoption. We believe broader adoption creates more long-term value than near-term revenue per procedure.
-- Jamie Samath, Chief Financial Officer
Surgeons are moving routine procedures to ambulatory surgery centers (ASCs), which are outpatient care facilities. Intuitive is capturing this shift by placing cost-effective systems into ASCs to block competitors from establishing a foothold.
If you look at the number of systems we've placed in ASCs last 2 quarters combined, that's more than we've placed in ASCs in almost the last 3 years, and we think that continues to be an attractive opportunity for us.
-- Jamie Samath, Chief Financial Officer
Management agreed to a $450 million litigation settlement covering third-party instrument remanufacturing and system servicing. The deal removes legal risk and cements long-term ties with major healthcare systems.
We believe that this no-fault settlement provides certainty for both our customers and Intuitive and maintains our ability to advance the safety, quality and security of our products.
-- Dave Rosa, Chief Executive Officer
Procedure volume is accelerating across international markets and outpatient centers, demonstrating demand across the entire platform portfolio. High switching costs and regulatory clearances secure its core competitive advantage.
Q3 was a strong quarter for Intuitive with broad-based adoption across our multiport, single-port and Ion platforms. In Q3, total procedures increased 15%, driven by 14% growth in da Vinci procedures and 31% growth in Ion procedures. In the U.S., da Vinci procedure growth was 10%, and outside the U.S., da Vinci procedure growth was 20%.
-- Dave Rosa, Chief Executive Officer
Management is watching pricing discipline and procedure expansion in routine surgeries. Combining proprietary hardware, software, and training networks cements its long-term market leadership.
Patients wait too long between detection and treatment and the care a patient receives still varies more than it should, depending on the hospital they enter and the surgeon available that day. Over time, we are focused on significantly advancing the Quintuple Aim to ensure that the broader patient journey can become more predictable, more connected and less disruptive to a patient's life.
