Deciding between established drugmakers requires balancing current income with future growth. You might be weighing AbbVie Inc. (NYSE:ABBV) against Novo Nordisk A/S (NYSE:NVO) to see which healthcare giant fits best in 2026.

AbbVie is evolving beyond its historic reliance on a single blockbuster treatment by investing heavily in immunology and oncology. Meanwhile, Novo Nordisk focuses on metabolic health, benefiting from a massive surge in demand for its obesity medications. Both companies operate in the competitive pharmaceutical sector but offer very different risk and reward profiles.

The case for AbbVie Inc.

AbbVie is a global biopharmaceutical player focused on developing advanced therapies for complex health issues. It competes among pharmaceutical stocks by focusing on high-impact treatments in immunology, oncology, and neuroscience. In its latest annual report, filed for the year 2025, the company highlighted a commercial strategy targeting physicians and government payers. Its U.S. distribution depends on three major wholesalers: McKesson Corp. (NYSE:MCK), Cardinal Health (NYSE:CAH), and Cencora (NYSE:COR). These three partners account for nearly all of its domestic pharmaceutical sales. Customer concentration like this adds a layer of risk to the business. The company also manages a growing aesthetics portfolio through direct-to-physician sales.

In FY 2025, revenue reached about $61.2 billion, representing a year-over-year growth of roughly 8.6%. The company reported net income of approximately $4.2 billion for the same period. This resulted in a net margin of around 6.9%, which measures how much profit the company keeps for every dollar of sales. While revenue increased during the period, net income saw a slight decline compared to the previous year. This trend reflects the ongoing transition as the company integrates new assets and navigates the loss of exclusivity for older products.

As of its December 2025 balance sheet, the debt-to-equity ratio was roughly negative 21.1x, which means total liabilities exceed shareholder equity. The current ratio stands at approximately 0.7x, which measures a company's ability to cover its short-term debts with its current assets. Free cash flow for the year was nearly $17.8 billion. Free cash flow represents the cash a company generates after accounting for cash outflows to support operations and maintain its capital assets. This strong cash generation supports the company's ability to fund research and development while returning value to its shareholders.

The case for Novo Nordisk A/S

In its latest annual report, filed for the year 2025, Novo Nordisk highlighted its role as a leader in metabolic health, specializing in diabetes and obesity treatments. It operates in roughly 170 countries and employs more than 69,000 people globally. The company focuses its research on chronic conditions like cardiovascular and rare diseases. While it does not disclose specific major customers in its filings, it maintains a vast global distribution network through various affiliates. This global reach allows the company to capture demand in both developed and emerging markets for its high-demand injectable therapies.

In FY 2025, revenue reached about $46.3 billion, an increase of approximately 6.4% from the prior year. Net income for the period was roughly $15.3 billion, resulting in a robust net margin of around 33.1%. This high net margin indicates the company is successfully converting a significant portion of its sales into profit. The growth in revenue was accompanied by a slight increase in net income, suggesting stable operational performance despite the costs associated with scaling its manufacturing capabilities.

As of its December 2025 balance sheet, the debt-to-equity ratio was approximately 0.7x. This ratio compares total debt to shareholder equity to assess financial leverage. Its current ratio was close to 0.8x, suggesting the company has nearly enough liquid assets to meet its immediate obligations. Free cash flow for the fiscal year was roughly $4.3 billion. This cash flow reflects the company's significant ongoing investments in manufacturing plants to meet the global demand for its weight loss and diabetes medications.

Risk profile comparison

AbbVie faces high revenue concentration, as Skyrizi and Rinvoq accounted for about 42% of 2025 sales. The company also deals with pricing pressure from the Inflation Reduction Act, which targets several key drugs for government-set pricing. Competition from generic manufacturers and biosimilars is also a major concern, particularly as Humira experiences substantial year-over-year revenue declines. Furthermore, the company must successfully integrate its $10.9 billion acquisition of Apogee Therapeutics. Ongoing complex litigation regarding intellectual property and drug pricing programs also adds uncertainty to its outlook.

Novo Nordisk faces its own set of challenges, including intense competition in the weight loss market from Eli Lilly & Company (NYSE:LLY). Changes in government drug pricing policies could also impact future profitability across its primary markets. The company remains subject to patent expiration risks, which could allow generic versions of its metabolic treatments to enter the market over time. Finally, maintaining the complex manufacturing capacity required for its popular injectable drugs remains a constant operational challenge. Any disruption in its global supply chain could limit its ability to capitalize on the high demand for its core products.

Valuation comparison

Novo Nordisk appears to be the more affordable option based on its lower sales and future earnings multiples. The Forward P/E ratio compares a stock price to future earnings estimates for the next year. Meanwhile, the P/S ratio measures the market value of a company against its sales over the past twelve months.

Which stock would I buy in 2026?

Novo Nordisk has the blockbuster drugs Wegovy and Ozempic (the same drug marketed for different purposes), but faces investor doubts about its market position given a deal with the federal government to lower prices for the GLP-1s, the emergence of generic competitors, and the apparent advances by Eli Lilly & Co. in developing weight loss drugs expected to be superior to Novo's offerings.

Still, the introduction of the once-a-day Wegovy pill in the U.S. has been a resounding success, and the business has a patent on the injectable Ozempic/Wegovy until 2032, providing it with a competitive moat. Still, Wall Street sees sales declining by about 3% in fiscal 2026, with net income contracting by 4% as pricing pressures slightly erode margins. That has led some analysts to label the company a "value trap." Long-term, analysts expect the company to reverse the 2026 decline and resume growth as new products come online.

AbbVie is buying itself growth with its recent deal to acquire Apogee Therapeutics. Its two relatively new immunology products, Skyrizi and Rinvoq, have proven to be true growth drivers for the business, and investors are hopeful that an FDA decision on a Parkinson's treatment later this year will bode well for the company. AbbVie is expected to see net income grow from $4.2 billion to $14.6 billion this year. Sales should grow to $67.2 billion.

AbbVie has a drug pipeline that some on Wall Street believe could include blockbusters (more than $1 billion in lifetime revenue) for treating schizophrenia, Parkinson's disease, psychosis from Alzheimer's, and certain solid tumors.

The temptation here may be to ride the GLP-1 boom and buy Novo Nordisk, but there are concerning headwinds for the business, even as it appears to be a relative bargain compared to AbbVie. AbbVie's expanding sales and income make it the better buy for long-term investors right now
