On its third quarter of 2026 earnings call, held on Nov. 4, 2026, Geron (NASDAQ:GERN) announced that full-year net revenue will reach the mid- to high end of its guidance range of $220 million to $240 million. Growing commercial adoption of RYTELO (imetelstat), an enzyme-inhibiting drug approved for lower-risk myelodysplastic syndromes (MDS), a group of bone marrow disorders, provides expanding cash flow from community oncology practices. Three things management said stand out.

1. Commercial Demand for RYTELO Continues to Expand in Community Clinics

Community oncology clinics drive the majority of patient prescriptions, giving Geron a growing footprint in second-line blood disease treatment. Net product revenue reached $60.1 million in the third quarter, representing a 27% increase year over year.

We're delivering consistent RYTELO demand growth, reinforcing our confidence in the low-risk MDS U.S. commercial opportunity and RYTELO's growth trajectory over the next several years.
-- Harout Semerjian, Chief Executive Officer

2. Discontinuing the Myelofibrosis Trial Eliminates Clinical Overhang

An independent monitoring committee found the Phase III IMpactMF trial unlikely to hit its primary endpoint for overall survival, prompting management to stop the study. Ending the trial avoids future clinical trial spending and sharpens focus on existing commercial priorities.

Last week, we announced the discontinuation of the IMpactMF trial evaluating imetelstat in relapsed and refractory myelofibrosis following a recommendation by the independent DMC. We believe this is the right course of action for patients and for Geron.
-- Harout Semerjian, Chief Executive Officer

3. European Launch Relies on a Gated Model to Preserve Cash

Geron is launching RYTELO in Germany by the end of 2026 without spending heavily on international sales teams. This staged approach maintains drug pricing overseas while leaving room to sign regional distribution partners later.

In Europe, our approach enables us to retain strategic control of pricing without having to establish full infrastructure in each market or commit to a large upfront investment. This hybrid model also allows us to remain flexible and open to partnership opportunities.
-- Harout Semerjian, Chief Executive Officer

Looking Ahead

Operational efficiency is helping reduce total costs, and management is openly pointing to improved expense controls. Operating expenses year-to-date fell 9% following earlier workforce reductions, while cash reserves reached $339 million. Management is watching commercial adoption rates, European reimbursement milestones, and disciplined capital allocation.

We are lowering our total operating expense guidance from $230 million to $240 million to the range of $220 million to $230 million.
-- Michelle Robertson, Chief Financial Officer

With a self-sustaining U.S. commercial footprint and substantial liquidity, management intends to leverage its existing infrastructure by acquiring additional hematology assets.

Beyond RYTELO, our financial strength gives us the flexibility to explore business development opportunities to build a leading hematology company.
-- Harout Semerjian, Chief Executive Officer
