  • Third-quarter revenue grew 19% year over year to $72.7 million.

  • Free cash flow reached $1.6 million during the third quarter.

On its third-quarter 2026 earnings call, held on Oct. 27, 2026, OrthoPediatrics (NASDAQ:KIDS) raised its full-year revenue guidance to $270 million to $273 million. Expanding adoption of specialized surgical implants gives the business durable growth across children's hospitals.

Three things management said stand out.

1. Product Super Cycle Drives Multiyear Revenue Opportunities

Management launched new pediatric surgical technologies, such as the 3P Hip plating system, which provides specialized implants tailored to children's anatomy. These new product lines command higher average selling prices and pull through demand for existing inventory already placed in hospitals.

The bottom line here is that we are still early in the multiyear cycle. And we believe the largest opportunities for market expansion, deeper portfolio penetration, revenue impact, and leverage are still ahead of us.
-- David R. Bailey, President and Chief Executive Officer

2. Non-Surgical Bracing Clinics Expand Total Addressable Market

The OrthoPediatrics Specialty Bracing franchise, or OPSB, expanded to over 60 clinics to treat pediatric orthopedic conditions outside the operating room. Bracing products like the DF2 femur brace don't require capital-intensive surgical instrument set deployments, generating fast cash returns and higher profit margins.

And with 60 clinics and 300 children's hospitals, there is an enormous runway of growth opportunities for us.
-- David R. Bailey, President and Chief Executive Officer

3. Disciplined Set Deployment Unlocks Positive Free Cash Flow

Management limited annual spending on surgical instrument sets to $10 million, focusing capital deployment strictly on high-return product lines. That restraint helped generate $1.6 million in free cash flow during the quarter, moving cash flow into positive territory after operating expenses and capital spending.

We continue to focus set deployment on higher return systems and remain disciplined in allocating capital to support growth.
-- Fred L. Hite, Chief Operating and Financial Officer

Looking Ahead

Procedure volumes are accelerating across children's hospitals, and management isn't hiding its satisfaction. Deep customer lock-in through specialized pediatric sets and growing clinic access give the business strong competitive advantages, while leadership watches operating leverage and sales force efficiency.

The combination of top line performance and improving profitability gives us a solid foundation as we enter our next chapter of growth.
-- David R. Bailey, President and Chief Executive Officer

As new product releases transition from limited trials to full commercial scale, the business continues prioritizing disciplined capital allocation and cash generation.

Overall, we are executing well against our strategy to translate strong revenue growth into robust financial performance.
-- Fred L. Hite, Chief Operating and Financial Officer
