On its third-quarter 2026 earnings call, held on Oct. 22, 2026, Molina (NYSE:MOH) reaffirmed its full-year adjusted earnings guidance of at least $5.25 per share. The company is managing higher medical costs across its government-sponsored health insurance plans. Rate adjustments from state partners are aligning with patient care expenses to stabilize underlying operating margins.

Three things management said stand out.

1. State Rate Increases Signal a Trough in Medicaid Margins

State reimbursement rates are beginning to match elevated care expenses, helping stabilize profitability across core Medicaid coverage for low-income residents.

The imbalance between rates and trend appears to have stabilized, which supports our view that current Medicaid margins represent the trough in a multi-year cycle.
-- Joe Zubretsky, President and CEO

2. Known Contract Backlogs Deliver $4 per Share in 2027 Embedded Earnings

New contract implementations, including the $6 billion Florida specialized children's health program, provide predictable profit growth as initial setup costs normalize.

We now expect to harvest $4 per share of embedded earnings in 2027.
-- Mark Keim, CFO

3. State Implementations of Work Requirements Show Moderate Member Attrition

Flexibility from state regulators in applying guidance from CMS, the federal Centers for Medicare & Medicaid Services, is limiting coverage disruptions and supporting steady enrollment.

As we work with our states and look at the range of possibilities, the range of flexibility that states have in interpreting CMS guidelines, I would say that on balance, 80% of our states are leaning toward policies that promote enrollment rather than disenrollment.
-- Joe Zubretsky, President and CEO

Looking Ahead

Reimbursement rates are catching up to patient care costs, and leadership sees operating margins stabilizing in government health programs. Disciplined bidding on RFP awards, which are competitive state contract proposals, alongside strong positioning in duals programs, plans serving patients eligible for both Medicare and Medicaid, provide structural tailwinds. Management is monitoring state rating discipline and medical utilization trends.

With the apparent plateauing of medical cost trend, which remains at elevated levels, and the strength of recent rate updates, we believe we are likely observing the beginning of the turn in Medicaid margins.
-- Joe Zubretsky, President and CEO

Contract backlog execution and expense efficiency build a clear bridge toward expanding earnings over the coming multiyear cycle.

These building blocks position us well for profitable growth in 2027.
-- Mark Keim, CFO
